BREEDING QUESTIONNAIRE

1.      Have you had litters with this breed before?  When, and how many times?

a. Please give details about the number of show vs. pet quality in each litter along with date of birth.  How was temperament and conformation assessed?

2.       Do you have contact information for all owners, which you have placed puppies with before?  This includes puppies you sold in place of a stud fee.  If they do not wish to be contacted, I will respect their request.  However, I would still like their information.

3.      How long have you been a member of the national breed club? Or regional Breed club?  Have you held any positions in the club?  If so, what were your goals during office?

4.      Did you sign the Breeder's Guidelines or Code of ethics? If not, what were your reasons that kept you from signing?

5.      Do you co own show puppies that are placed? If so, how are breedings handled?

6.      What are your requirements to show homes?

7.      What are your requirements to pet homes?

8.      Have you ever replaced or taken back a puppy? And for what reason?

9.      What is your biggest concern as a breeder?

10. Have any dogs been put to sleep in the line (any offspring of the grandfathers/mothers, please include littermates of sire and dam)? If so, what for?

11. How many puppies have you placed were incontinent?  At what age?  How was the situation handled with the puppy buyer?

12. How many puppies have you placed have bloated or have frequent licky fits?  At what age?  How was the situation handled with the puppy buyer?

13. How many litters has this Dam/Sire had and at what age(s)?   

14. Are both parents still living?   If not, what was the dog's age and cause of death?

15. How many puppies have you placed were diagnosis with pano?  At what age?  How was the situation handled with the puppy buyer?

16.      Do you have contact information for owners of littermates to the Sire and/or Dam?  .  If they do not wish to be contacted, I will respect their request.  However, I would still like their information.
